CME: Substance Addiction Diagnosis

From March 19 to 21, 2024, we hosted a Continuous Medical Education (CME) program focusing on addiction diagnosis for physicians. The trainees included 23 healthcare providers from various hospitals and healthcare facilities across the nation. Following the training, on March 21, the trainees took a quiz on topics learned from the program. Trainees received their CME certificates based on active engagement and quiz performance.

CME: Methadone Maintenance Treatment for Opioid Use Disorder (A Basic Course for Physicians)

From March 11 to 15, 2024, we conducted a Continuous Medical Education (CME) program which offered a basic training course on methadone maintenance treatment for physicians. The trainees included 42 healthcare providers from various hospitals and healthcare facilities nationwide. Following the theoretical training, trainees were required to complete a minimum of one week's practical experience at a methadone clinic and submit a case report. CME certificates were awarded based on participation and quiz scores, a prerequisite for physicians seeking to work at methadone clinics in Vietnam.

Web App ASSIST training: Yeboneers

The newly developed Web App version of the WHO ASSIST (The Alcohol, Smoking and Substance Involvement Screening Test) represents an important tool in the detection and provision of accessible intervention strategies for harmful substance use. The tool is used to improve the ability to map the levels of risk across various substances of use and misuse within the country and is thus set to reimagine effective passive data collection and mapping for substance use disorder risk across South Africa.

This training on the Web App ASSIST takes place in George, Western Cape, with 40 youth counsellors. These youth counsellors will be screening adolescents in schools.

V-ITTC presenting at Hanoi DSEP-Organized Conference

12/7/2023 - 12/7/2023
On December 7, 2023, the Hanoi Department for Social Evils Prevention (DSEP) organized a conference to evaluate the management, care, and support strategies for individuals post-drug rehabilitation within their residential communities in Hanoi. The Vietnam International Technology Transfer Center (V-ITTC) presented the results of their pilot intervention model, which aimed to enhance the social and legal assistance, consultation, and referral services for people who use drugs (PWUD) in the community. The project focused on improving the capacity and effectiveness of community social workers by adopting two evidence-based interventions: motivational interviewing and online support groups. The pilot model was well-received by community social workers, PWUD, and their families, showing some initial positive outcomes. V-ITTC will collaborate with Hanoi DSEP to continue and expand this model across Hanoi, focusing on varied communication channels and interventions tailored to the various needs of PWUD.

SA Central Drug Authority National Summit on Substance Use and Illicit Trafficking

About the Summit
The CDA has a mandate to host a summit wherein stakeholders engage on matters aligned to the use of substances in the country. The 4th CDA will host the 3rd summit in 2023, and it will be preceded by provincial dialogues.

The 3rd summit will consider a harmonised way to implement demand, supply and harm reduction in line with the National Drug Master Plan, i.e to support policy harmonization between health, social security and justice sectors, to support public health approaches for people who use drugs, people in correctional settings and other key populations.

The theme of the Provincial Dialogues and the SUMMIT is “Towards sustainable, coordinated and impactful strategies for national drug and substance use in South Africa”

University Walk-through Training on the Basic Level Series of the UTC

From November 9 to 11, 2023, the Vietnam International Technology Transfer Center (V-ITTC) at Hanoi Medical University organized a walk-through training on the basic level series of the Universal Treatment Curriculum for Substance Use Disorders (UTC). This training was conducted under the guidance of two regional trainers. The trainees included 21 national trainers from various universities, hospitals, and healthcare facilities nationwide. Following the training, on November 12, the trainees took the ICAP I exam, organized by the Colombo Plan in Hanoi, Vietnam.

African Union High Level Session on Addressing Substance Use and Related Mental Health Disorders

Objective

The overall objective of the meeting is to raise awareness and galvanise commitment and action of high-level Government leadership, policy makers, public health officials and other stakeholders to take appropriate measures (including legislative, administrative, social and educational etc) in addressing drug use prevention and SUDs treatment and related co-morbidities.

Specific Objectives of the high-level session
1. Enhance legal, policy and institutional frameworks to effectively prevent drug-use, crime and violence amongst youth
Sensitize for a comprehensive multi-sectorial health response to drugs. A response using the UNODC WHO International Standards on Drug Use Prevention and the UNODC WHO International Standards for the Treatment of Drug Use Disorders as guidance material for national and regional strategies in line with science focusing on human rights, gender-sensitivity and particularly vulnerable groups (children, women and youth)
3. Reflect on the overall health and social situation of drug use as well as experiences from the African Union that are in line with such standards in an effort to galvanize and network positive South-South experience exchange.
4. Discuss newly availed open-sourced tools designed for, and experiences derived from, Low- and Middle- Income countries in the field of prevention of substance use focusing on building family skills, life and social skills in a universal setting or in selective settings for higher risk groups (particularly those living in humanitarian setting).
5. Discuss newly availed open-sourced tools and services designed for, and experiences derived from, Low- and Middle- Income countries availing evidence­ based, affordable and accessible treatment services and systems in line with the human right principles. Such services will focus on alternative to convictions or punishment, treatment services geared for youth and their families as well as services for women including pregnant women. .
6. Advocate for the value of Youth empowerment and inclusion at the level of policy
making as well as·service provision and equipping them with the necessary tools, skills and platforms to become key enablers in preventing drug use, building resilience to drugs, crime and violence.
7. Mobilize Donor engagement and to commit increased resources for drug demand reduction interventions at the regional level and member state level
8. Agree on a roadmap for strategic interventions in addressing drug use prevention and SUDs among youth, women and children in African Union

Expected outcomes

1. Increased awareness and knowledge of drugs and drug use disorders and interventions in line with the UNODC WHO International Standards among High Level Government Leadership, policy makers, public health officials and other stakeholders
2. High Level continental dialogue stimulated to serve as a platform to promote
cooperation and collaboration to address evidence-based drug demand reduction responses
3. A roadmap for strategic interventions in addressing drug use prevention and SUDs
treatment among youth, women and children in the African Union agreed.
4. Donor engagement and expression of commitment and mobilisation of resources for drug demand reduction interventions at regional level and member state level.

Format of meeting

The high-level session will be held over three days, preceded by two days of senior expert meetings. Day 3 will culminate in the opening session and plenary interventions by Heads of State and Government on key meeting outcomes and the overview of SUDs and related mental health disorders on the continent. There will also be statements by the leadership f the AU, US State Dept.IINL, the World Bank Group and relevant UN agencies including UNODC, WHO etc.

A refresher course to update knowledge for the staff at the MMT facilities

From October 17 to 20, 2023, V-ITTC preventatives provided lectures during a refresher course for healthcare professionals from Methadone Maintenance Treatment (MMT) facilities, organized by the Hanoi Center for Disease Control. The course was specifically organized for treatment specialists, including over 100 doctors and counselors, who work closely with patients dealing with substance use or HIV. V-ITTC trainers led sessions on the latest knowledge in substance addiction, comorbidity, and chemsex screening and intervention.

Prevention for adolescents using synthetic drugs

From October to December 2023, the Vietnam International Technology Transfer Center (V-ITTC) teamed up with national broadcasting channels to produce and air a documentary series focusing on addiction treatment, community reintegration, and synthetic drug use prevention among adolescents. This series, consisting of four aired episodes and a live broadcast program, delved into the journey of recovery, the process of returning to society, and strategies for preventing adolescent synthetic drug use. The documentary emphasized the importance of family support and the involvement of multiple agencies in aiding recovery and prevention initiatives.

This includes the medication dispensing process, monitoring and management of methadone usage at home, and handling potential situations that may arise during medication dispensing and management.In addition to the knowledge gained from lectures, trainees were divided into practical groups, where they exchanged and listened to practical experiences shared by facilities that have implemented the program. Healthcare professionals emphasized positive results for patients, as not having to visit healthcare facilities daily has reduced travel time and expenses, providing favorable conditions for patients to access treatment while maintaining their work and daily life, ultimately improving the quality of life for patients.

Currently, the association has 36 member countries, with 10 Asian nations among them. The operational approach of each member within the association can be either an individual agency or a network. They hold regular quarterly meetings and organize workshops to share information and experiences in the field of substance use.

Web App ASSIST Launch

The South Africa National Minister of Social Development will on 29 May 2023 lead the dual launch of the National Department of Social Development Integrated Programme and the newly developed Web App version of the World Health Organization Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) (https://assistscreening.uct.ac.za).

This event launches two key initiatives in response to the call for integrated implementation of services in institutions of higher learning and colleges, especially, at the primary level of intervention by respective stakeholders to facilitate integrated early intervention and prevention programmes with specific focus on social crime prevention, risky substance use, and gender-based violence. The newly developed Web App version of the WHO ASSIST (The Alcohol, Smoking and Substance Involvement Screening Test) represents an important tool in the detection and provision of accessible intervention strategies for harmful substance use. The tool is earmarked to improve the ability of the country, at various entry points across health and social service sites, to map the levels of risk across various substances of use and misuse within the country and is thus set to reimagine effective passive data collection and mapping for substance use disorder risk across South Africa.

In 5 days from May 22-26, 2023, the Global Fund collaborated with the International Technology Transfer Center in Vietnam (Vietnam-ITTC) – Hanoi Medical University to launch a training course on Methadone maintenance treatment of opioid dependence for 50 trainees who are doctors and physicians working at health departments and methadone dispensing facilities from many provinces and cities in Vietnam - Bac Giang, Bac Ninh, Dien Bien, Hanoi, Hai Duong, Hai Phong, Lai Chau, Nam Dinh, Ninh Binh, Phu Tho, Quang Ninh, Son La, Thai Nguyen, Thanh Hoa, Vinh Phuc, Ha Nam, Cao Bang, Yen Bai, and Lao Cai. The group of lecturers comprised associate professors, doctors, and physicians from Hanoi Medical University, the Center for Disease Control and Prevention of Hanoi, and the Methadone Center of Dong Da district who are widely recognized as experts in the addiction treatment field.

Catalytic Drug Demand Reduction Meeting for South Africa Traditional Authorities and Leadership

The overall objective of the meeting is to understand public health problems in jurisdictions under traditional leadership due to increased availability and use of psychoactive substances trafficked into the continent and also produced locally, and to create a national network of Traditional Leaders in DDR in South Africa.
Specific Objectives of the meeting
1. Affirm the critical role of traditional leadership in DDR interventions.
2. Assess public health problems as a result of increased availability and use of psychoactive substances in areas under traditional governance in South Africa.
3. Explore the potential roles of Traditional Leadership in DDR interventions.
4. Assess needs of Traditional Leadership for effective engagement in DDR chart next steps.
5. Explore the possibility of establishing a national network of traditional leadership for DDR.
6. Agree a roadmap for the incorporation of Traditional Leadership into the DDR training programmes :the Universal Prevention Curriculum (UPC), Universal Treatment Curriculum (UTC), Universal Recovery Curriculum (URC)and into the other continental and international standards and best practices.
7. Agree a roadmap for enhanced systematic data collection to enhance evidence-based interventions and research by universities into substance use disorders.
8. Assess appetite and possibilities of establishing One-stop Service centres under Traditional leaders to assist drug dependent people.
9. Harness areas of synergy and facilitate active collaboration.
10. Acquaint Traditional Leaders with knowledge of drugs and drug use disorders and interventions

Central Asian open lecture series on Screening, Assessment, and Treatment of Substance use disorders

This series of lectures aims to bring together professionals from Central Asian region to discuss the topic of substance use disorders. They will cover basic topics on the main principles of understanding of substance use disorders, evidence-based backgrounds on the effective treatment and interventions of SUDs.

The scale of the conflict, its impact and persistence, together with gross violations of human rights in a multitude of settings, creates unprecedented challenges for mental health professionals in providing support and care to people affected by war.

For these reasons, we are making monthly consultation calls available to healthcare professionals who work with people in or from Ukraine affected by the war. Experts in helping people through the mental health consequences of mass violence and treating the mental health consequences of trauma will be available to consult on questions related to caring for people impacted by the war in Ukraine.

The purpose is to offer consultation on mental health related questions to healthcare professionals in Ukraine and abroad who are providing care to people affected by the war, to provide expertise on dealing with challenging situations in the course of their care, and create a platform to share experience.

Target audience: This consultation series is mostly targeting mental health professionals, but we invite any healthcare professional who would like to consult on questions related to the mental health of people directly impacted by the war in Ukraine.

Format: Since the consultation is interactive and generally does not involve presentations, we encourage participants to come with general questions (for example, «what to do about sleep disorders or stimulant use?», etc.) or with specific questions (for example, «what to do in a case where a child refuses to separate from their caregiver»). (We ask that you present questions in such a way that it is impossible to identify a specific person, so as to maintain confidentiality).

The duration of each consultation is 1 hour.
The number of participants in the consultation is limited, therefore it is necessary to register in advance. You will then be sent the meeting link via email.
All the events will be translated into Ukrainian.

Online workshop: “Guideline on Buprenorphine-to-Methadone Transition”

On the morning of August 31, 2022, the Vietnam International Technology Transfer Center (V-ITTC) – Hanoi Medical University in collaboration with the Department of HIV/AIDS Prevention and Control – Ministry of Health successfully organized an online workshop: “Guideline on Buprenorphine-to-Methadone Transition”. The meeting was joined by representatives from the Department of HIV/AIDS Prevention and Control, and Centers for Disease Control and Prevention of 8 provinces and cities that piloted the buprenorphine treatment for opioid use disorder program, staff from medical centers and drug-dispensing points, and Professor Gavin Bart from the University of Minnesota, Minnesota, USA. Dr. Bart is a Professor of Medicine at the University of Minnesota and director of the division of Addiction Medicine at Hennepin Healthcare. Besides his scientific achievements in the addiction treatment field in the United States, Dr. Bart has cooperated in conducting research and provided technical support for the Methadone treatment program in Vietnam from the very beginning.

Meeting to review the preliminary results of the pilot model for people who use drugs

On the morning of July 15, 2022, the Vietnam International Technology Transfer Center (V-ITTC), Hanoi Medical University in collaboration with the Department of Social Vices Prevention and Combat – Ministry of Labor, War invalids and Social Affairs held a meeting to review the preliminary results of the pilot model of social and legal assistance, consultation and referral services for people who use drugs. The meeting aims to discuss the results achieved by the model of social and legal assistance, consultation, and referral services for people who use drugs (PWUD) in the past time and propose an action plan for the next phase of the program based on new changes in the Law on Drug Prevention and Control of 2021.
The meeting was joined by 20 delegates from the Department of Science, Education, Culture and Social Affairs – Government office, Supreme People’s Court, Department of HIV/AIDS Prevention and Control – Ministry of Health, Department of Social Vices Prevention and Combat – Ministry of Labour, War Invalids and Social Affairs, the Center for Supporting Community Development Initiatives (SCDI Vietnam), and other international organizations including U.S. Department of State’s Bureau of International Narcotics and Law Enforcement Affairs (INL) and The US Center for Disease Control and Prevention in Vietnam.

Addiction Practitiners Forum

Substance-Use-Disorder (SUD) professionals often find themselves immersed in their work and as a consequence may not notice the ground-breaking developments which are happening around us. As initial-contact and treatment professionals, it is important that we invest sufficiently in our knowledge and understanding of new developments and the opportunities these may present.

For this purpose, the Addiction Practitioners Forum has been created by the South African National Council on Alcoholism and Drug Dependence (SANCA National) to close the gap between practice and theory. SANCA National has partnered the South Africa International Technology Transfer Centre (ITTC); International Society of Substance Use Disorder Professionals (ISSUP), South African Chapter and Clinical Governance to deliver this CPD initiative.

Addiction Practitioners Forum takes place on 1 & 2 July 2022 to explore, among others:

Capacitating Maternal Mental Health Champions in Motivational Interviewing for Behaviour Change.

Training participants include the Mums Support Network in KZN region of South Africa as well as SADAG (South African Depression and Anxiety Group), University of KwaZulu Natal and the DRILL (Developing Research Innovation, Localisation and Leadership) team.

Motivational Interviewing is a best-evidence approach for tackling harmful substance use and general motivation-related change, including treatment non-adherence. It is an approach that does not seek to replace other therapeutic approaches, but that seeks instead to support and strengthen them.

Managing Children & Adolescents with Addictive Disorders

This course provides students with an overview of risk and protective factors for child and
adolescent substance misuse, and discusses the prevention and treatment of substance use disorders amongst children and adolescents. Students learn about the normal stages of child and adolescent development, and how these may be affected by substance misuse. Students are exposed to lowthreshold evidence-based interventions for adolescents who misuse substances, as well as to ways to diagnose and effectively treat substance misuse among adolescents. Dual diagnosis, facilitating groups with adolescents, the impact of foetal alcohol spectrum disorders and prenatal methamphetamine exposure are also discussed.

Evidence-based Treatment Approaches

This course provides students with an understanding of evidence-based treatment approaches for
addictive disorders. Students are exposed to evidence-based principles of treatment and learn about the theoretical foundation, core concepts and principal techniques of several evidence-based psychosocial treatment models. Students are provided with intensive training in motivational interviewing and are expected to participate in role-play exercises. Basic training in cognitive behavioural therapy for the treatment of substance use disorders is included in the course. Other
treatment approaches including 12-step programmes, the Matrix Model, and harm reduction are also critically explored. The course is taught through lectures, practical demonstrations and role-play
exercises.

Screening and Assessment of Addictive Disorders

This course equips students to screen patients for problematic alcohol and drug use and conduct comprehensive assessments of the nature, extent and severity of alcohol- and other drug-related problems. Students are taught to use various screening tools and to effectively feed back these
results during brief interventions with patients. Students learn how to take a holistic patient history, conduct a mental state examination and present a concise etiological formulation. The practical application of urine testing in treatment settings is discussed. Students are taught principles and practical approaches to risk assessment, and clinical considerations relevant to alcohol and other drugs are examined. The implications that assessment findings have for patient placement and treatment planning are also outlined.

Understanding Addictive Disorders

This course provides students with an overview of alcohol and drug use both globally and in South
Africa, and the burden of harm associated with their use. Theoretical models for understanding
addiction are introduced. Students learn about the etiology of substance use disorders, as well as protective and risk factors contributing to their presentation. Barriers to treatment access are explored and students are taught about the neurobiology of addiction. An overview of classification systems for substances of misuse is provided and students are introduced to the range of interventions used to prevent initiation to alcohol and drug use, reverse the negative consequences of
use, and/or limit the harmful effects of alcohol and drugs where use continues.

Addiction treatment counselling (Basic)

The Vietnam International Technology Transfer Center, Hanoi Medical University in collaboration with the Department of Social Vices Prevention of Hanoi, opened a basic training course on addiction treatment counseling for more than 40 staff providing support and care for people who use drugs at religious rehabilitation centers from many provinces and cities across the country such as Bethel Catholic Rescue Center, “Nguon Phuoc (Source of Blessings)” Rehabilitation Center, and “Tai Sanh (Rebirth)” Rehabilitation Center.

Attending a 5-day-training course, from January 4 to January 8, 2022; learners are provided with knowledge and addiction treatment skills; in order to deliver optimal and successful addiction treatment services.

Treatnet Family Intervention Training Course

From 21st to 23rd of December, 2021; practitioners, social workers, and health workers from different parts of Vietnam have had an opportunity to participate in the 3-day online training course on Treatnet Family Intervention for people with substance use disorder operated by UNODC, The Ministry of Labour - Invalids and Social Affairs and Vietnam International Technology Transfer Center.

Family therapy involves a therapist and family members to improve and resolve relationship problems. Sharing feelings in family therapy helps family members improve communication skills and develop stronger family dynamics. Family therapy uses a range of counseling and other techniques including positive reframing, perspective taking and asking relational questions, and rolling with resistance. Providing family support through therapy can be an essential part of recovery for both the person with substance use disorder and their family.

"Universal Treatment and Prevention Curriculum" Sharing

Vietnam International Technology Center, based in Hanoi Medical University, Vietnam; has collaborated with DAP, Colombo Plan, and University of Medicine and Pharmacy in HCM city, Vietnam to organize a conference on "Universal Treatment and Prevention Curriculum".

This conference was held with the objective of emphasizing the role of DAP, Colombo Plan, and the collaboration of university partners in the implementation of the Universal Curriculum, as well as resources on substance use disorder treatment and prevention.

NRC is already a leader in promoting the adoption and implementation of science-based prevention, treatment and recovery services in UAE and throughout other parts of the Middle East. ICUDDR and the existing ITTCs are honored to include NRC in the ITTC Network.

About the ITTC Network:
The International Technology Transfer Center (ITTC) Network develops and strengthens the workforce, organizations and systems that provide substance use prevention, treatment, and recovery support services. Based in universities and research centers, ITTCs utilize a variety of strategies to accelerate the implementation of scientifically-based and culturally appropriate practices. ITTCs currently exist in Vietnam, Ukraine, and South Africa, with more countries being added in the near future. The Centers are brought together in a coordinated Network through the leadership of the ITTC Network Coordinating Office in partnership with the International Consortium of Universities on Drug Demand Reduction (ICUDDR).

Inclusion of UTC modules - Taras Shevchenko National University of Kyiv

Inclusion of UTC modules in pre and postgraduate curriculums in Taras Shevchenko National University of Kyiv. UTC 1-5 were included in curriculum on Work program of the discipline «Psychiatry, narcology» for 4th year medical students (knowledge field: 22 Health; speciality: 222 Medicine; education level: Master).

Date and number of hours: February-June, 2021. The total amount is 90 hours (20 hours – addiction psychiatry), including: Lectures - 10 hours (2 hours – addiction psychiatry); Seminars- 50 hours (18 hours – addiction psychiatry); Self-paced learning - 29 hours; Pre-exam consultation - 1 hour.
